There are leaders who are “legends in their own mind” (having been one, I know this first-hand).  They think they’re that darn good, never make mistakes, everyone loves them, and are blissful in their ignorance… until they’re no longer doing the job because they have been let go.  Hopefully, sometime in their career, they interact with another leader who can set them straight, and tell them that their poo does, indeed, stink like everyone else’s.  

Rarely, you’ll find a leader humble enough to know that they’re not that good.   They’re have situational awareness – it’s not an easy thing to keep the radar dish spinning in your head, so that you can be aware of everything happening around you, but it starts by walking around and talking to people at every level of the organization.  That doesn’t just mean “management by walking around”, although it includes that.  It means taking the temperature of your own leader and getting a feel for where your peers heads are.  Is someone under-water, and asking for a hand?   Give it – for if you just watch them sink, what are the odds that you’ll get a hand when you’re under water?
